Policy & Procedures


1. Walk-In Appointments: Walk-in appointments are accepted for sick patients only. Your child will be worked into the schedule as soon as possible; however, keep in mind that priority may be given to patients who are already scheduled.
2. Missed/Late Appointments: If you find that you will not be able to make your child's scheduled appointment, we ask that you call at least 24 hours in advance to notify us. Two (2) No-Call/No-Show appointments may results in a $25 service charge. Three (3) No-Call/No-Show appointments may result in dismissal as a patient from our practice. If you are more than 20 minutes late for your scheduled appointment, and we were not notified by a courtesy call, you may be asked to reschedule your child's appointment.
3. Identification: You, or your authorized representative, must provide a valid photo ID when bringing a child to our office.
4. Co-Pays & Fees: All insurance co-payments and office visit fees are required at time of service, unless prior arrangements have been made. If your insurance is not active when you check-in, you will be asked to pay the full office visit fee, plus any fees for testing or procedures done in office, or to reschedule appointments to another date when insurance will be active.
5. Triage Calls/Messages: Please remember our nurses are busy taking care of schedule patients. All attempts will be made to return calls and answer messages by the end of close that day. If you feel that the situation is urgent and can't wait, please make an appointment to see the physician.
6. Medication Refills: Please allow 48 hours for all medication refill requests to be transmitted to the pharmacy of choice. 
ADHD/Anxiety medications cannot be called or e-filed to any pharmacy. Patients must be seen every 2 months and will receive hard copy of prescription at appointment. Please allow 48 hours for call-in medication refill request for the month in between follow-ups.
7. Follow-Up Appointments/Physicals: Please understand that certain diagnosis and medications require follow up appointments to stay in compliance with child’s insurance. As well as being seen for their well child exams; at 1, 2, 4, 6, 9, 12, 15, 18, & 24 months and once a year thereafter.
8. Shot Records/Documentation: Our office policy requires 48 business hours’ notice for shot record or physical request. Any documentation needing to be filled out and signed by the physician may take up to 7 days to be completed.
9. Medical Records: As a courtesy, records can be forwarded to other physicians; however, for copies of medical records given to parents/patients there will be a $1.00/page for the first 25 pages and $0.25/page thereafter. Please allow up to 30 business days for medical records to be completed.
